Downloadable Voluntary Self-Identification of Disability Form

Allow Applicants & Employees to Identify Themselves as Having a Disability


As of August 4, 2020, businesses with 50 or more employees that hold federal contracts worth at least $50,000 must use a new form to satisfy OFCCP (Office of Federal Contract Compliance Programs) recordkeeping requirements. This form is part of their affirmative action obligations to allow job applicants and employees to self-identify as persons with disabilities, and to collect information to assess the effectiveness of their outreach and recruitment efforts.
